Northwell Direct is not an insurance company in the usual sense — it is a direct-to-employer network built by Northwell Health, which means your employer contracts with it and your cost-sharing is set by that employer's plan rather than by a national carrier's standard schedule. What stays constant is that we bill the plan directly, so you do not pay us up front and wait for reimbursement. Our PMH-C certified therapists treat postpartum depression, perinatal anxiety, birth trauma, and pregnancy loss, and they work exclusively in perinatal mental health. New York's parity and telehealth parity laws apply to Northwell Direct plans, so a video session is covered on the same terms as an in-person one. Because employer plans differ from one another more than commercial plans do, verifying your specific benefit matters more here than usual — we check your copay, deductible, and any prior authorization requirement before your first session, at no charge. Sessions are FSA and HSA eligible.

Does Northwell Direct cover therapy after a miscarriage?

Grief on its own is not a billable diagnosis -- it is a normal response to loss, and insurers do not treat it as an illness. Coverage attaches when the loss produces a diagnosable condition, which after a miscarriage is common: an adjustment disorder, a depressive episode, or PTSD where the loss was medically traumatic. In practice this means Northwell Direct covers the great majority of post-loss therapy, but the claim is filed under that diagnosis rather than under bereavement. We make that determination at the assessment, and it does not change what the sessions are like.

Pregnancy loss is one of the most under-witnessed griefs I see. People assume that early loss means small loss, and the data does not support that, and neither does the consulting room. The work is taking the loss seriously, naming it for what it is, and not letting the world's minimizing become your minimizing.

What therapy looks like

Therapy for pregnancy loss is grief-focused and paced to you. Many Phoenix Health therapists hold PMH-C certification, and several have specific training in perinatal loss. Modalities often include grief therapy, narrative work, and, when the loss was traumatic, EMDR or Trauma-Focused CBT. Early sessions usually focus on giving the loss room to exist. Many clients have been minimizing their grief, often without realizing it, because the culture around them did. The work begins by treating the loss as the real loss it is, and by helping you find words for what you're experiencing. From there the work might involve rituals or markers of remembrance, addressing relationship strain (partners often grieve differently and the mismatch creates its own pain), working through the fear of trying again, and processing any traumatic moments around the loss itself. Most people see meaningful change in 3 to 6 months, though many continue longer, especially through subsequent pregnancies.

  • There is no expiration date on this grief. The intensity usually softens with time and with support, but anniversary dates, due dates, and subsequent pregnancies often bring it back. That's not a failure of healing. It's how grief works.
  • No. Gestational age does not determine grief. Some people grieve a 6-week loss as profoundly as a later loss, especially if it was a long-awaited or hard-won pregnancy. Your grief is not measured in weeks.
  • Partners often grieve differently, and the mismatch is one of the most painful parts of this loss for many couples. Therapy can help bridge that, either individually or with couples work. The difference doesn't mean your partner doesn't care.
  • Many clients do exactly that. Processing the loss before trying again can dramatically change how the next pregnancy feels. That said, some clients start trying again before they've fully processed, and therapy can carry alongside that, too. There's no wrong order.
  • PMH-C (Perinatal Mental Health Certification) is awarded by Postpartum Support International (PSI) to clinicians who have completed advanced training in perinatal mental health — covering postpartum depression, anxiety, OCD, birth trauma, and related conditions.
